thumbnails displaying different image ?

the file number is the same the thumbnail has changed ?
anyone experience this before?
im not uploading new files with the same number and replacing the ones
i already have in there ?

Right Click on a blank spot on the page of Thumbnails.
Select ARRANGE ICONS BY....
Select NAME.
That should put all the thumbnails in their proper position.
